Please book the appointment using the parent's name exactly as it appears on the insurance card.​

 

After booking, please check your email and complete the forms sent to you. These forms must be completed 1 business day in advance. ​

 

The self-pay rate is a flat fee of $225.00 for an initial appointment and $100 for any follow-up appointments. Prenatal Education appointments are $175.00. If finances are a barrier, please reach out, as we do offer payment plan options. All appointments are virtual. 

 

Payment is due at the time of service. Super bill is available if out of network, please note we make no guarantee to any reimbursement by your insurance carrier. ​
