Billing/Insurance Information

Insurance Billing:

Often, a referral for lactation services is not required, but this can depend on your specific insurance policy, particularly HMO policies.

Many insurances cover our lactation services 100%, but billing through insurance does not guarantee reimbursement.

Since Emilie is in-network as an Adult Nurse Practitioner with the insurance carriers listed below, only the parent will have a claim submitted to insurance, therefore we require insurance information for the parent to be uploaded to our portal in advance.

 

Please notify us of any changes or updates to your insurance before your appointment.

Depending on your specific insurance plan, you may have a co-pay, deductible, or cost-sharing. If required by your insurance, a co-pay may be needed for the lactating parent at the time of the visit. We are legally obligated to charge these payments according to your insurance contract.

You may receive an Explanation of Benefits (EOB) from your insurance company. This is not a bill. If you have any questions, please contact our office directly.

 

You can also review outstanding balance information in your patient portal.

Self-Pay Rates for Unpartnered/Out-of-Network Insurances:

If you have a private insurance carrier that we do not accept, we can provide a superbill for you to submit for reimbursement. Please note, we cannot guarantee reimbursement.


The self-pay rate is as follows-

$200.00 for prenatal education appointments

$275.00 for an initial virtual appointment

$100 for any follow-up appointments.

Payment is due at the time of service. As required by law, all self-pay patients will receive a Good Faith Estimate upon booking.
